    College Application A.Y. 2023-2024

    The following are the requirements for SLSU College Admission AY 2023-2024:

    1. Duly accomplished College Application Form
    2. Clear copy of Academic Records
      • For Grade 12 students expected to graduate at the end of the Academic Year:
        Grade 11 Report Card duly signed by the principal and class adviser
      • For Senior High School Graduates:
        Grade 12 Report Card duly signed by the principal and class adviser; GWA of 83% or better
      • For Transferees and returnees:
        Transcript of Records (TOR)/ Copy of Grades with GWA. They must have a GWA of 2.25 or better.
    3. Certificate of income for BOTH parent(s)/ guardian(s)
      • For 4P's members:
        4Ps Certification from MSWD/DSWD indicating the Household Number and name of the beneficiary.
      • For Indigenous People Member ("Mga Katutubo"):
        Certification from the IP Chieftain
      • Enhancement of the Comprehensive Local Integration Program (E-CLIP) Beneficiary/ Payapa at Masaganang Pamayanan-Peaceful and Resilient Communities (PAMANA) Beneficiaries:
        Certification from DILG/ Office of the Presidential Adviser on the Peace Process (OPAPP)
      • Parents as business owners:
        BIR Form 1701A-2021
      • Parents employed in Government or Other Agencies:
        BIR Form 2316-2021 or Certification from the Human Resource indicating its salary and other compensation-related details
      • Parents working abroad:
        Overseas Employment Contract
      • With Deceased parents:
        Death Certificate
      • For Solo/Single Parent:
        Solo Parent Certificate from MSWD/DSWD or Notarized Sworn Statement indicating their current status (single, solo-parent, separated, etc.) and the amount and source of income.
      • Not classified from a-h (Unemployed, Housewife/ Househusband, no- fixed income, guardianship, self-supporting):
        Notarized Sworn Statement (indicating the name of Parents/Guardian Source and amount of income)
    4. 2x2 picture with nametag and white background (4pcs)
    5. Long white Folder (1)

    APPLICATION REQUIREMENTS SHOULD BE SUBMITTED TO THE CAMPUS OF YOUR FIRST CHOICE.
    Note: The link for the appointment will be posted on October 28, 2022 (for main campus applicants only). Be guided by the minimum health protocols in visiting the campuses. Applicants are expected to dress appropriately in casual clothes. Refrain from wearing any revealing clothes such as crop tops, tank tops, skirts, and shorts.

    PAANO BA ANG MAGSUMITE NG COLLEGE APPLICATION SA SOUTHERN LUZON STATE UNIVERSITY-MAIN CAMPUS (LUCBAN QUEZON)?

    1. Ihanda ang lahat ng mga dokumentong kailangan para sa College Application ay 2023-2024.
    2. Sagutan ang appointment link at hintayin ang confirmation sa email. SLSU College Application Appointment link:

    3. Magtungo sa SLSU Main Campus sa itinakdang araw at oras ng inyong appointment. Dalahin ang lahat ng dokumento.
    4. Isumite sa Student Admission Office ang inyong mga dokumento.
    5. Magsagot ng college application form sa itinakdang computer station ng Student Admission Office.
    6. Hintayin ang inyong examination permit sa Student Admission Office.
    7. Mag-exam sa itinakdang araw at oras.

    PAANO BA ANG MAGSUMITE NG COLLEGE APPLICATION SA SATELLITE CAMPUSES NG SOUTHERN LUZON STATE UNIVERSITY?

    1. Ihanda ang lahat ng mga dokumentong kailangan para sa college application ay 2023-2024.
    2. Pumunta sa satellite campus na nais pasukan (SLSU Alabat, SLSU Calauag Ext., SLSU Catanauan, SLSU Infanta, SLSU Dual-Tech Lucena, SLSU Gumaca, SLSU Polillo, SLSU Tayabas, SLSU JGE-Tagkawayan, SLSU Tayabas)
    3. Isumite ang inyong mga dokumento.
    4. Magsagot ng college application form sa itinakdang computer station ng satellite campus.
    5. Hintayin ang inyong examination permit.
    6. Mag-exam sa itinakdang araw at oras.
