We are still accepting online applications but make sure to take note of the schedule of tests in your municipality because this is your last chance to take the SLSU College Admission Test for AY 2020 -2021 (if application is made after the schedule in your municipality, your application will be VOID):
Please secure the following requirements to be uploaded during your online application:
- Valid Gmail Account
- Scanned copy of colored 2″x2″ photo with nametag taken during the last six (6) months
- Scanned copy of:
- Grade 12 Report Card for AY 2019-2020 (back-to-back)
- TOR for returning students and transferees who have not yet taken the SLSU College Entrance Examination in the last two (2) years
- Proof of income of both parents/guardian:
- For Members of 4Ps or Indigenous People (IPs)
- Scanned copy of Municipal Social Welfare and Development (MSWD) Certification
- Scanned copy of 4Ps/IPs I.D.
- For Non 4Ps or IPs Members
- FOR EMPLOYED: Scanned copy of Certificate of Tax Withheld (BIR Form No. 2316) for year 2019
- WITH BUSINESS: Scanned copy of Income Tax Return for Individuals, Estates and Trusts (BIR Form 1701) for year 2019
- FOR OFWs: Scanned copy of Overseas Employment Certificate (OEC) – latest issuance or Employment Contract indicating the salary of employee
- OTHERS: Scanned copy of Certificate of Tax Exemption from BIR for the year 2019
- For Members of 4Ps or Indigenous People (IPs)
